Job Description

Flexible working hours Monday to Friday. This role is to support people through social care processes,help them to participate and have a voice, supportingatmeetingsand helping themto understand the information that is being shared with them. You will be obtaining and sharing a person’s views with health or social care professionals involved in making best interest decisions on their behalf.This role includes supporting people of all ages. You will be working with people with dementia, people with learning disabilities, autistic people, people with physical and sensory impairment, people with an acquired brain injury and those with mental healthneeds. Please note,this role does not involve supporting people with their personal care and you are not required to workshifts, evenings or weekends.What is the day-to-day of the role:

  • Providing face-to-face support to people who lack capacity to make decisions about where they live, medical treatment, information being shared with them and be involved in planning their future care and support.
  • Representing people who are subject to the Deprivation of Liberty Safeguards.
  • Supporting all ages with various learning disabilities and additional needs.
  • Keeping up to date, accurate records in line with Data Protection anddemonstrate a good working knowledge of legislation, policy and procedures around the Mental Capacity Act, Care Act and wider legislations.
  • Working in the community and attending meetings with professionals.
Required Skills and Qualifications:
  • Must have full driving licence and access to own vehicle for working hours.
  • Must have advocacy experience.
  • Must live within the County Durham area.
  • Have experience working in the Health and Care sector and with people who have additional needs.
  • Have excellent communication and negotiation skills both verbally and in writing.
  • Good IT skills and be proficient using Microsoft Office 365.
  • Be passionate and resilient.
  • Able to work on own initiative and maintain own calendar and databases.
