Rushcliffe PCN is developing a primary mental health practitioner team that will both part replace the traditional general practitioner role (being first contact for mental health presentations and managing primary care appropriate mental health conditions) and also operate effectively across the secondary care and third sector interface. We are seeking a lead for this new team. The ideal candidate will be an experienced Mental Health Nurse who is dynamic, looking for a new challenge and committed to working in an innovative way to develop a new and forward-thinking integrated team within General Practice. You should have significant experience of working in mental health services and have experience of inter-agency working. You will be skilled in undertaking comprehensive assessments, risk assessments, delivering brief intervention care planning, including physical health care, health promotion, social inclusion and recovery
