An exciting opportunity has arisen to work as part of a Multi-Disciplinary team to offer clinical leadership and caseload management within a nursing home setting, forming part of the discharge pathway from the acute hospital setting.You will be based at Rusticus Care Home in Cotgrave, Nottinghamshire, but will be employed by Nottinghamshire Healthcare NHS Foundation Trust, and see patients from the Greater Nottinghamshire locality. You will offer clinical leadership to a multi-disciplinary team focused on high quality patient care and enablement, working together to discharge patients to their appropriate place of care.
