Interested in a new opportunity in Nursing Look no further than Leeds Community Healthcare (LCH) NHS Trust.The Integrated Children with Additional Needs (ICAN)-service are seeking to appoint an experienced, highly motivated and enthusiastic Registered Nurse with community nursing experience, to join our existing nursing team. This is an exciting time for ICAN as we continue to develop a seamless service for children and young people with additional needs and their families. We are proud of all we achieve in the ICAN service and in order to fulfil our vision we are recruiting a permanent full-time band 6 ICAN Community Nurse (suitable for a Registered Nurse with Family Nursing, Health visiting, School Nursing or Paediatric Nurse background).We are looking for a dynamic, passionate nurse with experience of caseload management including safeguarding and multi-disciplinary working with excellent communication skills, leadership and organisational skills and the ability to motivate and support others through change.Key responsibilities include working with children and their families who attend our Community Paediatric Clinics - supporting care delivery and nursing interventions for those children and their families. You will work within our nursing intervention clinical pathways including sleep, behaviour, continence and autism with a commitment to achieving the best possible outcomes for children with additional needs, including those with neuro-disability. There is an expectation that you will contribute to the co-ordination and support of the childrens strategy locally in order to develop co-ordinated pathways with other health and social care partners.We can offer exciting opportunities for staff who share our commitment and vision who are willing to collaborate in order to achieve the very highest standards of patient care. Whilst you will be based at one centre predominantly, you will be required on occasions to cover across the city.
