Job Description

Service Coordinator (Supported Housing, Complex Needs)Location: CreweSalary: £18870Hours: 37.5Contract Type: PermanentOur client is looking for a Service Coordinator to join their Cheshire East team on a full time permanent basis to provide tailored support to help their customers live independently in the local community.Job requirements:

  • Provide emotional and practical support to customers in the community. Referring them to external services when needed.
  • Attend customer review and progress meetings.
  • Work towards service requirements, targets and objectives.
    • What they’re looking for:
  • Experience supporting adults, young people or families with complex needs like substance use, homelessness and mental health.
  • An understanding and compassion for people without a home, with an understanding of the barriers they face.
  • Great communication skills.
  • Able to problem solve to get the best outcome for everyone involved.
  • Someone who is flexible and adaptable and can put the needs of our customers first.
  • Someone who can work evenings and weekends.
  • The successful person for this role will need to be DBS checked too.
