Job Description

Cheshire East17.5 hours per weekFixed term until end of March 2023£10,591 per annumOur client is looking for someone to join their Reconnect service helping over 55s regain confidence and reconnect with their community. Youll be providing practical support to vulnerable people via home visits, signposting and providing emotional supportthrough phone calls.Job requirements Support customers to rebuild their confidence by helping them with practical skills like shopping, collecting prescriptions, meal preparation, completing forms, sending and receiving mail and dealing with bills. Connect people with their communities by supporting them to attend activities, groups, facilities and services. Address loneliness and social isolation by re-building links with friends, family, neighbours and the community. Provide low level mental health support to rebuild confidence, resilience and self-esteem. Signpost and advocate to specialist support providers such as Snow Angels, Wishing Well, Age UK, CAB, Carers Hub, NHS, Social Care and DWP.What theyre looking for Someone with experience of delivering support to customers in their homes. Someone who knows the services available to support people living in the Cheshire East area. Good ICT skills to use their digital case management system. Great communication skills to develop honest and genuine connections. Someone who is organised and able to plan their own time well. Someone who is confident and resilient, able to think quickly to problem solve and use their initiative to work independently. Life experience that allows you to show compassion to provide the very best support. Someone who has their own vehicle suitable for business use.
