An exciting opportunity has arisen for a Clinical Specialist Occupational Therapist (temporary fixed term post of 1 year) with clinical skills and experience of working with Stroke patients to join a dynamic, developing Community Stroke service, based at Eagle Bridge in Crewe.This would be an ideal post for an individual who is passionate about Stroke and who would like to further develop and apply their specialist skills in Community Stroke. Although the band 6 post is a rotational post, the temporary fixed term post will be based in the Specialist Community Stroke and Rehabilitation Team.The Community Stroke Team provide specialist stroke assessment and rehab as part of ‘Early supported discharge’ and longer term rehab services for up to 4 months following discharge form hospital.We work collaboratively to deliver goal focused, person centred, integrated therapy to individuals and their relatives in their own homes and residential / nursing homes in the Crewe/Northwich and surrounding areas.The Community Stroke Team consists of a wide range of professionals from PT /OT to dietetic services, Speech and language therapists, nursing, psychology services, social workers and rehab assistants and works in partnership with stroke association and other community services locally to provide a holistic service to the patient.There are current plans for Community Stroke Services Development which could provide further opportunities in the future.This is great opportunity for someone who shares our passion for good quality patient care and enthusiasm for ongoing improvements, working with patients and our partners to design our services better together.* Travel is essential requirement of the postEvery effort will be made to accommodate special interests and all staff will receive supervision/ support and be encouraged to participate in continuing professional development.
