This exciting position is for someone who has excellent interpersonal and communication skills and is able to lead andmotivate people.About the Waiting Well ServiceThe service will offer support to individuals who have been waiting two years or more for elective procedures or treatment from the NHS and, through continued communication and reassurance, reduce the risk of reliance on emergency, acute and primary care healthservices and empower people to better self-manage their physical health and well-being as they continue to wait for treatment.It will work in conjunction with NHS and Local Authority services enabling people to engage with their local communities, which would otherwise prove difficult due to frailty or anxiety.Our primary objectives are to:* Support the service user whilst they await treatment and/or admission to hospital* Improve service user health and well-being* Facilitate or provide practical help in their own home* Connect service users with community-based services to support their well-being and reduce their anxietyGair am y Gwasanaeth Aros yn IachBydd y gwasanaeth yn cynnig cymorth i unigolion sydd wedi bod yn aros dwy flynedd neu fwy am driniaeth ddewisol gan y GIG a, thrwy barhau i gyfathrebu a thawelu meddyliau pobl, bydd yn lleihaur risg o ddibynnu ar wasanaethau iechyd brys, acwt a gofal sylfaenola grymuso pobl i hunanreoli eu hiechyd au lles corfforol yn well wrth iddynt barhau i aros am driniaeth.Ein prif amcanion yw:* Cefnogi defnyddwyr y gwasanaeth tra byddant yn aros am driniaeth a/neun cael eu derbyn ir ysbyty* Gwella iechyd a lles defnyddwyr y gwasanaeth* Hwyluso neu ddarparu help ymarferol yn eu cartref eu hunain* Cysylltu defnyddwyr y gwasanaethgwasanaethau yn y gymuned i gefnogi eu lles augwneud yn llai pryderusDuties include:* Regular What Matters conversations with service users to establish if there are any additional support needs to maintain independent living and enhance their quality of life* Carrying out an informal assessment of the service users wider non-clinical needs to establish if their immediate needs are met (safe home environment, basic food supplies, hot water, heating, etc.)* Signposting and supported referral to other agencies and third sector partners including Falls Prevention, Care & Repair, Carers Support, Age Cymru, Alzheimers Society etc.Within the proposed Waiting Well service model staff and volunteers will not provide personal care support.Bydd y dyletswyddaun cynnwys:Sgyrsiau rheolaidd am yr Hyn syn Bwysig gyda defnyddwyr y gwasanaeth i ganfod a oes ganddynt unrhyw anghenion cymorth ychwanegol i ddal ati i fywn annibynnol a gwella ansawdd eu bywydCynnal asesiad anffurfiol o anghenion anghlinigol ehangach defnyddwyr y gwasanaeth er mwyn canfod a yw eu hanghenion uniongyrchol yn cael eu diwallu (amgylchedd cartref diogel, cyflenwadau bwyd sylfaenol, dwr poeth, gwres, ac ati)Cefnogaeth emosiynol fel eistedd gyda rhywun neu siarad dros y ffnrhywun syn ddryslyd, mewn trallod, gan eu galluogi i drafod eu pryderonTrefnu siopa a gwasanaethau eraillNodi a gweithredu ar unrhyw bryderon diogeluCyfeirio ac atgyfeiriochymorth at wasanaethau eraill yn y gymuned syn gallu helpu unigolion i gadwn iach gartrefCysylltugwasanaethau lleol eraill y Groes Goch Brydeinig, fel gwasanaethau Cymhorthion Symud a swyddogion cyswllt y Trydydd SectorCyfeirio ac atgyfeiriochymorth at asiantaethau eraill a phartneriaid yn y trydydd sector gan gynnwys Atal Codymau, Gofal a Thrwsio, Cymorth i Ofalwyr, Age Cymru, Cymdeithas Alzheimers ac ati.O fewn y model gwasanaeth Aros yn Iach arfaethedig ni fydd staff a gwirfoddolwyr yn darparu cefnogaeth gofal personol.To be successful in this position you should be the following:Full driving licence holder and access to vehicleAbility to deal with queries in a diplomatic, professional and confidential mannerUnderstanding of how to improve service quality for the benefit of usersThe closing date for applications is 23.59 on Wednesday the 29th of June 2022 with interviews to follow.
