Job Description

Advantage Healthcare is a leading provider of complex care across London and the South EastWorking in partnership with NHS continuing healthcare teams and case management companies our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of care required is highly complex.We deliver care 24/7, 365 days a yearWe are dedicated in providing care for those patients who are too ill to look after themselves or be looked after by carers or family members and who would otherwise be admitted to, or have to remain in hospital, but who do not require 24-hour nursing care.We are looking for an ITU or Registered nurse with extensive experienceof tracheostomy/ventilated community based clients who are prone to being very unstable sought to join a very establishedteam of professionalsfor a male client in the North West of London to work long days and night shifts.In addition to the Tracheostomy/ventilation- he also needs support with cough assist, bowel management, peg, clinical obs. , suctioning,catheter care, bed-bound so full care needed with an HCA 24/7 for assistance with manual handling and bowel management.What we offer:

Some of the other duties required will be:
  • Lead assessment, care planning and evaluation of patient care.
  • Communicate effectively with patients, carers and all members of the multi-disciplinary team.
  • Prioritise daily work in line with clinical/patient need.
  • Maintain accurate and legible patient records in line with professional and organisational requirementsthis includes contemporary paper records and electronic patient record systems.
  • Actively participate and contribute to team meetings, handover and MDTs.
  • Initiates health promotion activities, as appropriate, with individuals, patients and carers.
  • Participate in risk assessment and promote the safety and well-being of patients, staff and any other persons at all times and assist in ensuring a safe work environment.
  • Ensure that all complaints, untoward incidents, accidents and areas of clinical risk are identified and reported to the office/Branch Nursein accordance with the organisation policies.
  • Keep up to date with professional requirements, health care developments, clinical evidence, National Service Frameworks for practice, National and Local priorities.
  • Responsible for completing all mandatory training.
  • Responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintainedincluding equipment loaned to patients.
  • Supervise care delivery from Non registered staff e.g. HCAs.
For this position we would also require:
  • At least 3 years ITU/HDU post registration experience- tracheostomy, Ventilation, Bowel management, Nebulisers, observations, oxygen, cough assist, chest physio, catheter care, peg care and medication management. Dealing with very unstable patients.
  • Experience of community working( not essential)
  • Experience of working as a member of a team with other professionals, but also able to work autonomously
  • Registered General Nurse (Adult)
  • Valid NMC registration
  • Experience of teaching and assessing students and other staff members( not essential)
  • Documented evidence of continuing professional development
  • Knowledge of the principles of clinical governance and its application
  • Awareness of assessing/managing/preventing risk in the working environment
