Job Description

Are you an enthusiastic, motivated and compassionate Band 5 Occupational Therapist looking to join the West Somerset Integrated Rehabilitation Team (IRT) based at Minehead Community Hospital in Minehead covering the ward and the community of West Somerset.The West Somerset Integrated Rehabilitation Team (IRT) forms part of the larger Integrated Community Services Team consisting of IRT, District Nursing and Older Peoples Mental Health covering Williton, Minehead and the West Somerset community. We are currently looking to expand our community links working in a more collaborative way with patients, other agencies and the voluntary sector within our local ‘neighbourhood’.The Occupational Therapists provide person centred rehabilitation and discharge planning to the ward, and continue the goal focused approach into the community. The team consists of Occupational Therapists, Physiotherapists, Rehabilitation Assistants, and Senior Rehabilitation Assistants with access to Pharmacy Technicians.This position will provide the opportunity for you to develop holistic cognitive, environmental & functional assessment skills in the community hospital and surrounding community, across a variety of conditions. You will also develop your rehabilitation and clinical skills, deepen your knowledge base and strengthen your clinical reasoning in a structured, supportive environment.As part of this role you must have outstanding communication and team working skills, and the ability to inspire and motivate your patients to maximise their physical and functional abilities such that they can take these forward on discharge from our service.
