Job Description

Do you have a passion for supporting patients Are you a highly motivated and dynamic individual who would relish the opportunity to work within a range of community teams Then this could be the ideal role for you.An exciting opportunity has arisen for a Band 6 Occupational therapist to join our band 6 rotations. in this role you will spend 12 months within each specialty Primary Care Network, Frailty Team , Neurology and Early Intervention Vehicle.The Integrated Community and Neurology Team - The team supports patients with long-term neurological conditions across the Great Yarmouth and Waveney area, and consists of specialist nurses, physiotherapists, Occupational Therapists and rehabilitation assistant practitioners. Assessment and intervention is completed in clinic settings as well as in patients’ homes and care homes. We provide timely assessments and treatment to facilitate and improve patients’ quality of life and maximize independence. You will take responsibility for a designated caseload of patients, supporting them throughout their journey from initial diagnosis, management and palliative care.Primary Care Homes - The Team support patients in the community with complex care needs who require specialist physiotherapy input to aid in function and mobility. Supporting patient to achieve optimal independence. You will be working as part of the MDT teams which include, Nurses, Occupational therapists, paramedics, rehabilitation support works, assistants practitioners, therapy assistants pharmacy support, health care assistants and community matrons.Frailty Prevention teamThe Community Frailty Prevention Team is a multidisciplinary team consisting of, Occupational Therapists, Physiotherapists, Assistant Practitioners, Therapy Assistants and Nurses.The team can provide a variety of interventions which may include: a home safety and environment assessment, strength and balance training, medication review, coping strategies or confidence building, equipment provision or referral to other professional agencies, voluntary organisations or community support if necessary.The Team will endeavour to advise patients, relatives and carers on how best to assist patients to remain well and manage their everyday living to maximise independence and to support to remain at home. The team works Monday to Friday 8.30 – 4.30.EIVThe primary role of the Early Intervention vehicle is to provide holistic and clinical assessment to enable people to stay independent and to continue to live at home more safely, where prior an acute admission may have been necessary.Offering benefits to patients such as - • Immediate provision of assistive technology equipment • Immediate provision of adaptive equipment such as toilet seats, commode• Immediate access to walking aids• Rapid assessment to extra support or care provision through community and social care provision infrastructure• Improved access to community provision / pathways • Promotes independence• Immediate Pendant alarm provisionWorking a shift pattern of 3 12 hour shifts (fulltime) over 7 days therapists will support our patients to remain and home and will allow follow up by Frailty Prevention Team where appropriate.The successful applicant will be able to demonstrate a high level of clinical expertise, as well as excellent communication and organizational skills. We provide a structured supervision program, including clinical support and regular in-service training. In return, we expect applicants to show a commitment to Continued Professional Development and evidence based practice.
