Working hours: 37.5 hours per weekWorking in Intermediate Care is an exciting opportunity that you may never have thought about before! We have 59 beds across two Intermediate Care Units based in Ellesmere Port Hospital. It is our aim to deliver person centred care, in the right place, at the right time.We are looking for enthusiastic highly motivated newly qualified or experienced Band 5 nurses to join our Intermediate Care Team’s.We would welcome applications from both newly qualified nurses and those with experience with an interest in care of older people. We are seeking Individuals who are comfortable to challenge change, and are committed to improving patient experience.The Intermediate Care Units have a different focus on care and provide a person centred approach with the aim of optimising physical health and wellbeing over a 24 hour period. Both units’ provide short term rehabilitation for patients who are medically optimised but not at a suitable level for discharge home. Patients will transfer from emergency medicine, current inpatient beds and repatriation for a period of rehabilitation.The nursing role will be to support patients to manage their long term conditions and act as an autonomous practitioner, as the units have limited Medical cover. Normal nursing duties include; medicine administration, wound and pressure care, discharge planning and patient education.Due to the complexities of our patient group we require nurses to closely monitor patients, ensuring patients remain suitable for rehabilitation and concerns are escalated accordingly.This role would develop your autonomy, discharge planning skills and relationships with community and 3rd sector services alongside being in a proactive and supportive team.For further details / informal visits contact:Name Ali WylieJob title Lead NurseEmailTelephone number
