Job Description

Job Title: Registered Nurse - Bridgewater CommunityTrust Location: WarringtonPay Rates: £14.92, £19.40, £23.88 + HolidayShift Patterns: Days, Nights, Weekends & Bank Holidays As a Community Nurse working in Warrington you will be integral to the care of the patients during their time with Bridgewater Community Trust. The Trust is focused on supporting the Community and helping patients remain independent in their own home. Youwill actively support the District Nursing team by undertaking responsibility for a delegated caseload. Undertake the assessment, planning and evaluation of patients care needs. Liaise with the District Nurse team to ensure information sharing in relationto patient care / changes to care plan and the continuity of care and reviewing of complex care packages. What youll be responsible for
  • Promoting the health and wellbeing within the family unit and community as a whole.
  • Performing comprehensive assessment of patient nursing needs as the named nurse.
  • Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
  • To support the team leader in the co-ordination of complex and continuing care packages.
  • Maintain accurate records in accordance with NMC.
  • Attend a multi-disciplinary case conference as the named nurse if appropriate.
  • To communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
  • To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
  • To competently perform clinical skills related to the post.
  • To be professionally responsible for a delegated caseload under the supervision of the District Nurse.
  • To assess health needs at individual, practice and community level.
  • To undertake chronic disease management reviews using appropriate template.
  • To work without direct supervision
The role will involve, but not be limited to you performing the above.Youll need to have the following qualification/experience:
  • A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years
  • Experience with Wound Care, insulin and medication administration, catheter care and palliative care.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices
  • Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
  • Undertake regular mandatory training
