Job Description

Clinical

  • To be responsible for a clinical caseload and ensure that the residents receive appropriate treatment to a high professional standard, in accordance with the Professional Standards and Code of Ethics of the College of Occupational Therapist (COT). To beprofessionally and legally accountable for all aspects of own work.
  • To undertake a specialist assessment of residents, including those with diverse or complex physical disability and multi-pathologies; use clinical reasoning skills and manual assessment techniques to provide an accurate foundation for the therapeutic managementof resident’s conditions.
  • To prepare and deliver a resident-centred intervention based on a sound knowledge of evidence-based practice and treatment options, using specialist clinical assessment, reasoning skills and treatment skills. These would include 24-hour postural management,resident education, cognitive programmes / activities, ADL and IADL among other intervention options.
  • To be responsible to undertake and guide junior occupational therapist specific / comprehensive multi-disciplinary and profession specific assessments utilizing investigative skills for those with complex neurological needs e.g. pre-admission assessments,functional assessments, cognitive assessment reviews with other professionals, neurological assessments of upper and lower limb motor and sensory function, postural and wheelchair seating assessments, assessment for and provision of mobility equipment, medicinesmanagement, cognitive assessments, home assessment and community assessments.
  • To be responsible to undertake a highly specialist assessment of neurological occupational therapy which includes cognitive assessments for PDoC such as The Wessex Head Injury Matrix (WHIM) Sensory Modality Assessment and Rehabilitation Technique (SMART)and or The JFK Coma Recovery Scale—Revised (CRS-R) and other cognitive assessments such as The Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MOCA). Also ability to use and interpret assessment results for further therapeutic management.
  • To able to provide tailored occupational therapy / meaningful activity program on a wide range of intervention options and to monitor, evaluate resident’s progress with appropriate and valid outcome measures to formulate the resident-centred care throughframework / model of care e.g ICF
  • To ensure competence in the use of all required equipment and are used safely at all times during treatment; and to ensure residents’ competence in and safe use of any appliances and aids used by them.
  • To be responsible to demonstrate knowledge, skill of using outcome measures, e.g SMART,GAS goals, FIM/FAM, NPTDA-PHYS, NPDS-H, NIS, RCS, ASIA, pain scales, cognitive assessment battery and other necessary measurements. Also ability to use and interpret assessment results for further therapeutic management.
  • To liaise with internal and external professional e.g GP, Physiotherapists, Occupational therapists, wheelchair therapists, Consultants and other staff from local organisations such as the CCG (Clinical Commissioning Group) and Social Services for residentseffective management.
  • To be responsible for the provision of Assistive Technology to residents including power mobility, ADL / IADL equipment and other alternate positioning equipment as needed. And closely monitor resident’s status and assist therapy staff with modifying equipmentto best meet the resident’s needs.
  • To be responsible to represent the occupational therapy service and or individual resident, whenever necessary, eg at the multi-disciplinary team meetings, to ensure the delivery of a co-ordinated multidisciplinary service, and integrate occupational Therapyintervention into the treatment programme. This will include discussion of residents care, progress and involvement in discharge planning.
  • To be responsible to contribute to delivering a high quality, efficient and effective service at all times complying with all relevant policies, procedures, guidelines and legislation.
  • To be responsible to develop an understanding and utilising of community resources/services to meet resident-centred needs and to ensure the community resources/service information list is updated.
Education and Training
  • To be responsible for maintaining his / her own competency to practice through CPD activities and maintain a portfolio which reflects personal development.
  • To be responsible to maintain and develop current knowledge of evidence-based practice in the area of working, developing specialist knowledge of particular condition(s).
  • To be an active member of the in-service training programme and clinical audits by leading, attending and participating in sessions. To attend individual training sessions, external courses and peer review as appropriate.
  • To be responsible to provide training to assistants, including assessing and ongoing evaluation of therapy assistants’ performance.
  • To be responsible to provide training to caregivers (such as HCAs), nursing staff and residents’ families e.g 24-hour postural management, transfer techniques, handling skills, ADL, IADL walking.
