Job Description

South West London and St Georges Mental Health NHS Trust has a rich history, well-established community links and an international reputation. When you join us, you’ll be part of something special – an innovative and rapidly growing organisation that is helping to redefine the Mental Health, education and social care landscape in the UK.Based at Tolworth Hospital the Kingston Community Mental Health Teams comprise of two multidisciplinary teams covering North and South Kingston. They provide community care to adults with mental health difficulties who are registered with GP’s in the Kingston Borough. The Trust was rated ‘Good’ by the CQC for our Community Services and we are looking to improve to ‘Excellent’ so we need a motivated and ambitious workforce to help us to deliver efficient, safe, caring, responsive and well led services.We are looking for clinically skilled nurses, occupational therapists and social workers to take up the vital role of Care Co-ordinator in the teams. The post holder will hold a caseload and work with a varied, complex caseload of mental health clients. The role primarily involves developing treatment and interventions of health conditions within a recovery model of care but successful candidates will need to demonstrate an understanding of people’s social care needs.The aim is to enable service users to work towards their goals and use the available interventions, offered as part of their specific treatment plan, to then be transferred to primary care services or to access tertiary services, if necessary.As part of the role the post holder would have the opportunity to mentor and supervise nursing students in their relevant fields.The role would also include supporting operational functions within a depot clinic and possibly with the local well established Clozapine clinic.Staff Development and Support:As part of the care co-ordinator role, we also have the following specialist training opportunities:Structured Clinical Management (SCM):Kingston services have recently developed an exciting new Personality Pathway. SCM is an evidence based approach for general mental health professionals with people with personality disorders. It is a structured approach which aims to support people to develop problem solving skills, manage crisis and develop skills to manage their emotions, using both individual and group work. You would receive training in SCM and have regular supervision with an experienced team.High User Network/Serenity Integrated Mentorship (SIM):SIM (Serenity Integrated Mentoring) is an award winning mentoring programme for high intensity users who are struggling to cope which often leads to frequent detention under Section 136 of the Mental Health Act.SIM trains Police Officers in mental health intensity behaviours, risk management and basic clinical theory. There is an officer based within the Kingston teams who works with identified service users, alongside the care co-ordinator. The aim is to offer consistent support and mentoring to enable the service user to develop more helpful skills and strategies to support with recovery. You would receive training in SIM as part of the SIM London project.The teams have monthly reflective practice and regular individual supervision is offered. There is a strong commitment to developing our staff in order to deliver high quality care. There are a variety of lead roles for staff to take, which provides an opportunity for further development in specific areas of interest.Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK. This is easily accessible via the Northern Line (Tooting Bec Station) and via private transport. The Trust provides subsidised parking fares for all our substantive staff.
