Gaining experience across low secure and locked rehabilitation environments to support better health and greaterindependence.Reporting directly to the Regional Physical Healthcare Lead, you will empower recovery, health and wellbeing as you create one-to-one rapport with service users where you will be responsible for monitoring the physical health and undertaking assessments.As a Physical Care Coordinator, you will create and maintain working relationships between The Woodhouse and local GPs to provide primary health and development treatment plans.A range of training opportunities are available that can see your career grow and you achieve your career aspirations.Your responsibilities

  • Monitor and maintain Physical Health Dashboards
  • Work in collaboration with the Registered Nurses to ensure the Hospital Passports and Heath Action Plans are kept up to date
  • Maintain up to date clinical records in line with the electronic records system
  • Support the Nursing team to monitor maintain medical emergency equipment at the site
  • Prepare Physical Health Summary report for any internal or external meetings
To be successful in this role, youll need
  • Previous experience within a similar environment
  • Experience working with people with learning disabilities and autism.
  • Advanced qualification and or experience in working in primary care settings
