Community Rapid Intervention Service (CRIS)Advanced Practitioner (Nurse/Paramedic/Therapist)Band 71.0 WTEs In collaboration with Midlands Partnership Foundation Trust (MPFT), the University Hospitals of North Midlands (UHNM) are embarking on a pioneering journey to support the patients with acute rapid response care within their home preventing hospital attendance and/or admission. Supporting the NHS long term plan to bring care closer to home we are now ready to launch this innovative service that will see a MDT approach to supporting patients in their home providing acute care and medical management plans to those patients deemed appropriate in line with our governing criteria. We are therefore looking for keen, enthusiastic and dynamic practitioners who feel they have the autonomous skills to provide care to patients in their home. Candidates will be clinically credible having both health assessment and preferably non-medical prescribing attributes within their portfolio of practice. Candidates will join this service in its infancy and so service transformation and development skills will be welcomed alongside strong leadership skills and excellent clinical practice.Understanding the need for innovative practice and transferable workforce these posts are unique and open to applications from registered nurses, Paramedics and therapists.If you feel like you would like to be part of this service development and would like to join our Team at this exciting time informal discussion and/or visits are encouraged, in return applicants will be supported with a robust governance framework, opportunities to develop and be supported with clinical supervision.This service will be provided initially between the hours 08.00hrs-22.00hrs 7 days per week and so applicants will be expected to cover these hoursFor any further information please contact Lesley Roberts Senior Clinical Operational Manager on
