  • Providing knowledgeable, compassionate, and trusted advice, guidance and support to people affected by dementia.
  • Helping people understand dementia and how to cope with day to day living whilst preparing for the future.
  • Assessing the needs of people affected by dementia.
  • Offering advice and support to your clients whether this is in the clients own home, at service offices, out in the community, face to face, phone, letter, or email.
  • Provide information, emotional and spiritual support to people living with dementia, their carers and families who are either planning for later end-of-life stages or reaching end of life.
  • Raise awareness of the benefits and importance of planning for the future and making advanced care decisions.
  • Work with multi-disciplinary team members to support people with dementia to discuss end-of-life wishes and create an Advanced Care plan record which can be accessed when needed.
  • Provide information to professionals and families to understand/interpret people presenting with non-verbal communication and behaviours arising from dementia
  • Provide information and support to increase the confidence of local Dementia Advisors when initiating end-of-life discussions
  • Work with Care Homes and other professionals to raise awareness of dying well with dementia
  • Work with the multi-disciplinary team help to develop a local end-of-life pathway
  • Work with the multi-disciplinary team to develop an end-of-life resources pack
  • Assist the person living with dementia to record advanced end of life care planning.
  • Skill to carry out an evaluation of the service on a quarterly basis.
  • Attendance at all relevant external meetings to represent the views of the Society
  • Competence to help record the wishes for end-of-life care for the person living with dementia.
  • Able to inform professionals in the palliative care field of the service.
About you
  • Qualified to NVQ level 3 (or equivalent) with an understanding of dementia and the needs of those living with dementia and their carers.
  • Ability to offer specialist holistic person-centred End- of- life information support and signposting to relevant partners to people with dementia and their carers
  • Able to take the lead within the local team on palliative care.
  • To keep updated in all end-of-Life policies procedures, research and training.
  • It is desirable you have experience of coordinating others such as volunteers.
  • Experience of developing relationships and liaising with statutory and voluntary agencies.
  • Be able to travel independently across North Staffordshire.

  • Excellent interpersonal skills, being approachable and friendly.
  • Excellent communication skills, both verbal and written.
  • Great organisational skills to manage your caseload of clients effectively.
  • Excellent IT skills and ability to use Microsoft packages confidently.
  • Be able to work collaboratively with other team members if/when required.
