Thats when it hits home.We have a fabulous job opportunity for you to join our awesome team as a Support Coordinator.We support our brilliant customers who havealcohol recovery and/or substance misuse support needs. They live in the South Bank area of Middlesbrough in a supported living accommodation.We help our customers enjoy the best quality of life they can, support them to maintain social inclusion and access and understand benefits and budgeting. We also support them to access voluntary work, local services or day centres and support them to liaisewith relevant stake holders and support services.Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers. Each plan will lookto meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.Youll also have responsibility for coordinating our support workers, apprentices, and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.We empower our customers to develop the independent living skills they need, to successfully move on.
