Easington District Primary Care Network (PCN) is seeking an additional, enthusiastic and proactive pharmacist to develop as PCN Clinical Pharmacist working as part of the practice multidisciplinary team.A part time post is available (0.5 whole time equivalent) working in the East Durham Medical Group (this is a large practice covering multiple locations including Wingate, Peterlee, Horden and Wheatley Hill).You will be self-motivated, able to work independently and above all will be passionate about providing the highest standards of care and treatment to patients in primary care.You will work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing. Perform face-to-face structured medication reviews of patients with polypharmacy; especially for those with frailty and/or with multiple co-morbidities as well as undertaking reviews of patients with specific long term conditions that fall within your competency. You will also contribute to quality improvement and clinical audit, as well as supporting aspects of the Quality and Outcomes Framework (QOF), medicines safety and antibiotic stewardship.In addition to the patient facing aspect requirements of your role, you will also be required to contribute towards practice financial and quality performance. You will work to deliver medicines optimisation initiatives within GP practices including the provision of specialist professional pharmaceutical advice and services to ensure the safe and cost effective use of medicines. This will involve work to deliver key medicines optimisation outcomes against a CCG set work plan.Ideally, you will have an independent prescribing qualification and clinical examination skills or will be willing to work towards them.Mentorship will be provided by our Senior Clinical Pharmacist and by the GP(s) within the practices and the successful candidate will be expected to enrol on or have qualified from an accredited training pathway (currently, the Clinical Pharmacist training pathway: CPPE Clinical Pharmacists in General Practice Training Pathway or CPPE Medicines Optimisations in Care Homes Training Pathway).The successful candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.Experience in General Practice would be an advantage but is not essential if the applicant is clinically up-to-date and understands the process of a full medication review in the primary care setting.This is a fantastic opportunity to be mentored and upskilled across a number of GP practices to achieving a greater clinical role for the future.If this role appeals to you and you have the qualities we are looking for then please get in touch:Dr Dagny Samuel, Clinical Director, Easington District Primary Care Network and GP East Durham Medical Group: emailInterview date to be confirmed.
