Job Description

Lead Community Link Worker (FTC)Forth Valley Royal Hospital (with travel across the Forth Valley area)Full-Time, Fixed Term ContractOverviewBelonging to the Royal Voluntary Service community means inspiring volunteers to support the nations health and wellbeing. They are also some of the countrys most compassionate and dedicated people who step forward to help make society better connected and more inclusive- where everyone can thrive.The RoleAs the Lead Community Link Worker, you will manage the new Community Wrap Around Discharge Service for Forth Valley.Working with all partners, including NHS Forth Valley, Dial a Journey, Strathcarron Hospice, Falkirk HSCP and Clackmannanshire & Stirling HSCP and the Food Train, you will develop the service to meet the needs of the patients at the Forth Valley Royal Hospitalto make sure discharges happen as quickly and safely as possible.In this role, you will be responsible for:- Acting as the first point of contact for referrals for the service- Managing the delivery of activities associated with the Winter Pressures Partnership- Networking and building relationships across all areas- Working within the Partnership to ensure that the needs of the patients are at the heart of the serviceWorking HoursThis role is a fixed term contract until 31st March 2023, working 35 hours per week. Please note, some weekend and evening work may be required.What youll needTo join us as the Lead Community Link Worker, youll need:- Experience of managing and motivating teams of staff and volunteers- To be commercially aware with relevant experience of managing a similar service type- An understanding of how to use basic data to monitor trends and provide forecasts- Excellent communication skills- Proficiency in IT- A full, valid driving licenceWhat you get in returnWere proud to be in the top 25 not-for-profit organisations to work for in the UK, based on ratings from Indeed.
