Are you looking for a clinical role which will see you drive forward innovation, deliver skill mix change and push boundaries Then you might want to come and have a chat with us! We are looking for a salaried GP colleague to join us in shaping and influencing our future growth and service innovation. As well as undertaking the clinical aspects of the role (which is delivered through a mix of triage, face to face appointments, video, online and telephone consultations and home visits) we are looking for an individual who will support us in driving our practice vision further forward. The current partners and staff have invested a deal great of time and energy to create a dynamic and forward thinking environment, but also somewhere that is enjoyable and professionally rewarding to work in. Weekly teaching sessions are in place for our clinical staff which either comprises of case reviews, internal sharing of learning or an external speaker and all clinicians are encouraged to support these sessions as well as daily virtual meetings for hot case reviews and colleague support.Since 2014, our team has evolved and we have recruited into both established and novel posts breaking down artificially established barriers, encouraging self directed, developmental innovation and a senior supportive environment. We are multi - disciplinary in our approach and the team consists of Advanced Nurse Practitioner, Nurse Practitioner (specialising in Well Woman and Diabetes), 2 Pharmacists, 4 practice nurses and 3 HCAs (and of course 3 GPs into the mix!). We are currently recruiting for an Advanced Clinical Practitioner. We are one of two practices in Wales to employ a palliative / frailty nurse. We enjoy close working relationships with our Community Nursing, Health Visiting, Midwifery Teams as well as excellent links with the 3rd sector and Local Authority colleagues. Working across 5 sites, we are building links with the community we work in (circa 10,500 patients) through the practices but we also serve a community hospital, with admission rightsWe have developed links through offering training placements for Medical and Physician Associate Students and we plan to develop academic links in addition to the teaching role. We also run a Medically Assisted Recovery programme based at the YMCA in Swansea.
