Job Description

We are seeking to recruit motivated and caring staff that are committed to providing high quality, person centred care and support and who are looking for a career in social care. Our service within the locality of Droylsden in Tameside enables people to live as independently as possible in their own homes and be fully part of their local neighbourhoods.Within your role you will provide an integrated health and social care package to older people living in their own homes in Droylsden. You will act as a case manager ensuring service users have an up to date support plan which meets their holistic needs. You will carry out all identified social care tasks and those healthcare tasks delegated by health professionals such as District Nurses. You will ensure a flex outcome focused service which is responsive to changing needs. You will work creatively to reduce social isolation and enhance peoples lives through addressing loneliness and link people in with a range of appropriate community activities. You will also provide direct care and support to our service users to ensure all their needs are met in a dignified way.If you are creative and feel you could work with people on a one to one basis to build positive relationships and enable individuals to live fulfilling lives, building on strengths, skills and interests then we would love to receive an application form from you.The service will operate from a Community Hub in Droylsden, which will offer a range of activities for the people we support and be accessible to the local community.We offer excellent training and opportunities for careers progression, as Creative Support is a national provider of services for people with a range of needs. You will undertake the Care Certificate and Diploma in Social Care and receive additional specialist training to enable you to work effectively with people with a range of needs. We can offer you a part time contract of up to 22.5 hours per week, either Saturday or Sundays or both.Candidates must have a clean driving licence.This role is exempt from Section 7 2e of the SDA.
