It is with excitement that following recent investment and re-evaluation of our Physiotherapy service in Llanelli – the biggest town in Carmarthenshire, Hywel Dda University Health Board presents these vacancies.We are committing as a service to improve our acute-community rehabilitation pathway collaboratively with the MDT to bridge the gap between the primary and secondary settings. As well as joint working session with occupational therapy (OT).The successful candidate will be working across the acute and community settings to facilitate the integrated rehabilitation pathway in Llanelli. Working closely with the MDT attending weekly consultant led meetings, ward rounds and board rounds. Caseload consisting of complex rehabilitation patients with discharge planning needs. Working closely with the Acute medical team identifying patients appropriate rehabilitation.Admission avoidance in the community and managing patients presenting with frailty syndrome will also be a big part of the role.
