General Practitioner – Intermediate CareAt Hywel Dda University Health Board we are starting to do things differently and are seeking to enable our GPs to work in new ways.Would you like· To shape and clinically lead intermediate care pathways across our region· Have an impact and improve the quality of life for our older population· The opportunity to work alongside a dynamic multi-disciplinary team· Flexibility to ensure a work life balance· To be able to pursue a portfolio careerAs part of our transformation journey we are seeking to appoint GPs to join our community multidisciplinary team and contribute to the design and delivery of intermediate care services. Specifically, we are hoping to recruit experienced General Practitioners with an interest in intermediate care both in a patient’s pre-crisis phase and also when presenting with an acute crisis. The aim is to ensure that where admission can be avoided, that medical and/or multidisciplinary care provision is available. Further, where acute hospital admission is necessary, that patient’s discharge to the community is facilitated as efficiently as possible to a community setting - either to their home or to an intermediate care facility.Successful candidates will join a growing cohort of similar clinicians and the community multidisciplinary team and contribute their medical expertise to a broad range of Health Board’s Urgent Emergency Care developments as outlined below:· Urgent primary care pathfinder as Clinical Navigators based in acute hospitals to undertake triage of presenting patients whose needs could be supported within primary care and community services· Intermediate care response alongside multidisciplinary teams to support admission avoidance and / or early discharge particularly for frail older patients.· Clinical triage with Welsh Ambulance Services NHS Trust (WAST) and mitigating conveyance to acute hospital where appropriate.· Same Day Emergency Care working alongside secondary care consultantsSessional commitment variable and to suit you, very happy to discuss individual needs and wants. Shift loading to reflect unsociable hours. Regular educational input, appraisal and ongoing training needs will be met.
