Fixed term / Secondment until 31st March 2022Would you like to be part of a small dynamic team who work in partnership across the region to improve outcomes for our population Have you got experience of working with or within BAME communitiesAn exciting opportunity has arisen to join a small outreach team, supporting communities that are facing inequalities in health outcomes. The outreach team will develop close links to local authorities, Third Sector and community organisations to raise awareness and understanding of Test Trace Protect (TTP) and will support wider health messages, especially those thought to be contributory factors to disproportionate adverse outcomes for individuals e.g. BAME communities.The outreach team will also support communities to have a greater understanding of their rights and access to health care, as well as playing an important role in identifying trusted community intermediaries and establishing mechanisms to sustain future engagement.The post holder will be expected to deliver a community outreach service across Pembrokeshire, Ceredigion and Carmarthenshire.The post holder will develop strong links with trusted intermediaries and key groups and be able to identify key community influencers and establish on-going relationships to support continuous engagement activities.The post holder will be based in one of the three counties of Carmarthenshire, Ceredigion or Pembrokeshire. The service base will be located at Prince Philip Hospital with a requirement to travel to various locations across the wider Health Board area.The successful post holder must be a team player but with the ability to work autonomously and prioritise to be able to deliver to agreed deadlines.
