Fixed term / Secondment until 31st March 2022Would you like to be part of a small dynamic team who work in partnership across the region to improve outcomes for our population Have you got extensive experience of working with or within BAME communities Do you have experience of managing othersAn exciting opportunity has arisen to join a small outreach team, supporting communities that are facing inequalities in health outcomes. The outreach team will develop close links to local authorities, Third Sector and community organisations to raise awareness and understanding of Test Trace Protect (TTP) and will support wider health messages, especially those thought to be contributory factors to disproportionate adverse outcomes for individuals e.g. BAME communities.The outreach team will also support communities to have a greater understanding of their rights and access to health care, as well as playing an important role in identifying trusted community intermediaries and establishing mechanisms to sustain future engagement.The post holder will be responsible for the management, development and evaluation of the Community Development Outreach Team across Pembrokeshire, Ceredigion and Carmarthenshire.The post holder will deploy available resources to ensure the delivery of community outreach and review delivery against service objectives. They will work collaboratively with Health Board, Local Authority, community based teams and other key stakeholders.The post holder will be based in Prince Philip Hospital but may be required to travel to various locations across the wider Health Board area.The successful post holder must be a team player but with the ability to work autonomously and prioritise to be able to deliver to agreed deadlines.
