Job Description

Mediplacements are looking for an experienced band 7 Permanent Pharmacist to work at a hospital in Kent.The pay for this position is £40-£47K per annum, and we are looking for someone who can start as soon as possible for a permanent position.Job IntroductionWe are looking for a passionate Ward Pharmacist to work across our Adult Community Services, which includes 4 inpatient rehabilitation wards, community nursing services and specialist nursing services for long term conditions. You will be part of our Quality & Patient Safety Team who are based in Kent. Kent is one of our four sites and you will be required to travel across our other sites in Gravesend, Sittingbourne, and Sheppey. As part of the role, you will need to have a validdriving license and access to a car to visit the relevant sites. This is a part-time (30 hours/4 days) opportunity where we offer flexible working in line with the needs of the service. Your working hours can be fulfilled during the working day which is between 8am and 6pm with set days being confirmed at interview basedon preference. The Ideal CandidateTo be successful in this role you will be a registered pharmacist with GPhC and have a minimum of 3 years post qualification experience. You will deliver a high quality, patient-centred clinical pharmacy service across the four-community hospital inpatient wards by providing a clinical service which includes prescription reconciliation, medication review, reordering of prescribed medicationswhere necessary and supporting safe and timely patient discharge and organising discharge medications for TTAs . You will work as a member of the MDT to ensure the delivery of a high-quality clinical service by participating in the functioning of our inpatient areas. Main ResponsibilitiesAs the Ward Pharmacist you will offer support and guidance to our Adult Community Services in North Kent and the responsibilities will include but are not limited to:* To provide a comprehensive clinical pharmacy service to the four-community hospital inpatient wards (Livingstone; Sapphire; Harty and Kestrel wards). * To ensure compliance with medicines legislation, support nursing and medical staff and assist with education and training, to ensure safe, effective, and economical use of medicines. * To work with nursing colleagues and ensure that adequate stock levels are maintained.* To ensure adequate supply of medicines are available when patients are admitted.* To support patients to self-manage medication providing education and training as required. * To support discharge planning activities and organise medicines supply where needed.* To work with the Lead Pharmacist to share best practice across services, promoting consistency and efficiency in clinical processes.When you work with Mediplacements, you will have access to a range of benefits that will support you in your role.
