South Somerset West PCN is looking for a Physician Associate to work in an acute primary care setting, linking in with the Clinical Assessment Service and to help support admission-avoidance into secondary care and reduce pressure on primary care.The post holder will be a trained Physician Associate, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients through (but not limited to) clinics, clinical triage, on-call duties, home visits and relevant administrative work, ensuring the highest standards of care for all registered and temporary patients of SSW PCN .You will be supported to develop this new role including opportunities to support research, service evaluation and quality improvement. This role would suit an individual who wants to work with an innovative and collaborative team keen to share ideas and new ways of workingYou will be a team-player, who works effectively in an MDT clinical setting.
