Job Description

We are looking for an experienced social worker to join our Home Safe Hospital Discharge team in Northumberland. An exciting opportunity has arisen for an experienced social worker to join our enthusiastic Home Safe Hospital Discharge Service across Blyth and the Northumbria Specialist Emergency Care Hospital, which will also include facilitating out of area hospitaldischarges from North Tyneside and Newcastle hospitals. The posts will be 37 hours per week on a permanent basis.Whilst the post will predominantly be based at one site, we will require the successful candidate to be flexible in providing service cover at other hospitals across the County.About the role We are passionate about keeping vulnerable adults safe, while respecting and protecting their individual needs, aspirations and integrity, both mental and physical. You will work alongside and in partnership with individuals, families and carers using a range of interpersonal and communication skills to comprehensively assess complex psychosocial need, set objectives, develop care plans and deliver appropriate interventionsto facilitate safe hospital discharges.Hospital Discharge isnt easy, but its effectiveness is its own reward and supporting people to access the correct level of support when they leave hospital is rewarding. Our social workers and care managers always remember that people are the experts of their own lives, and it is our role to work alongside them to identify strengths-based and outcomes-focused solutions. We work in a way that enhances individual involvement, choice and control as part of improving quality of life, wellbeing and safety. We are looking for someone with driveand determination to make a difference, the ability to maintain and forge positive relationships with colleagues in the Primary and Secondary health is important. Essential requirements: Degree in Social Work (or equivalent) Be registered with Social Work England Have a minimum of 3 years post qualifying experience A thorough knowledge of Safeguarding Adults. Ideally you will be working in the field of adult social work, be able to demonstrate an understanding of the Care Act, Mental Capacity Act and know how to applySafeguarding thresholds and criteria.
