Our brand new CQC registered service provides support to our brilliant customers who havelearning disabilities and physical health problems / disabilitiesand are living in the community. Our customers live in afour-bedroom bungalow with their own room andshared facilities with 24 hour staffing in Choppington, Northumberland.Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll help our customers identify their aspirations, and work with them to create plans that helpthem achieve these. Youll create bespoke and personalised support plans with and for your customers.Each plan will look to meet our customers individual potential and long-term goals, making sure they meet safeguarding and confidentiality requirements. Youll also help your colleagues to deliver on the plans youve created to achieve excellence. Our aim is to support our customers to live their day to day lives as independently as possible.Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.You cansee what its like to work in a service like thisinthis great video.The team will tell you that no two days are ever the same because everything we do is person centred around our customers.
