Job Description

About The Role The Chelmer Primary Care Network(PCN) which comprises of 4 Practices operating from Chelmsford City is looking to recruit a full time clinical pharmacist to support patient care in line with the new Network Contract Directed Enhanced Service (DES) Specification.Main purpose of job: To support the effective delivery of patient care in GP practices within a definedPrimary Care Network (PCN) by utilising pharmaceutical skills to embed the principles of medicines optimisation within the wider practice teams, ensure safe patient care through regular review and monitoring of patients based on clinical need and support demand management through consultation with patients as appropriate. Clinical Care

  • Run clinics where medicines are the main interventions e.g. anticoagulant monitoringclinics.
  • Conduct spirometry, child immunisation, travel vaccination, diabetic foot checks andNHS Health Checks as well as other roles as required on completion of appropriate training.
  • Conduct care planning including mental health, admission avoidance schemes and Endof Life patients.
  • Review daily Pathology results and act on results.
  • Undertake minor ailments triage and direct patients based on clinical need to capturerelevant patient health information at all consultations including contributing to the quality outcomes framework.
  • Monitoring patients with complex long term conditions such as hypertension, diabetesand COPD, ensuring that medication is optimised, titrated, monitored and complied with.
  • Providing clinical advice and expertise on medication.
  • Establishing ongoing professional relationships with individual patients.
  • Act across the interface liaising with other providers i.e. secondary care, communityPharmacy and care/nursing homes as necessary to ensure safe transfers of care.
  • Deliver on and/or oversee relevant aspects of the PCN Directed Enhanced Service (DES).
  • Take a central role in the clinical aspects of shared care protocols, clinical research withmedicines, liaison with specialist pharmacists
  • Be part of a professional clinical network and have access to appropriate clinicalsupervision.
  • Support the PCN clinical director as necessary
  • Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
  • Review prescribing against local and national targets and engage with the wider prescribing team to rectify.
  • Carry out structured medicines optimisation reviews in care homes
  • To contribute to the reduction of medicine wastage within practices across the PCN.

The right candidate will have a passion for ensuring quality, excellent interpersonal skills and will lead the medicines optimisation agenda on behalf of our primary care network.Ideally the suitable candidate will have a qualification from an accredited training programme for primary care based clinical pharmacists, which includes registration as an independent non-medical prescriber. The right candidate should demonstrate the attributes, values and behaviours to support this newly developing network. You will be a qualified Pharmacist with strong experience within Primary Care and have the following skills / qualifications:
  • Degree Pharmacy, and registration with the General Pharmaceutical Council
  • Ideally you will hold or be working towards a postgraduate pharmacy qualification
  • Minimum of 2 years post qualification experience in pharmacy (hospital, primary care or community).
  • Working towards/being an independent prescriber
  • Member of the Royal Pharmaceutical Society (RPS)
  • You will have awareness of the breadth of common acute and long term conditions that are likely to be seen in a general medical practice
  • Be able to demonstrate general pharmaceutical skills and knowledge in core areas
  • You will be ableto plan, manage, monitor, advise and review general care plans for patients with long term conditions
  • Be able to demonstrateaccountability for delivering professional expertise and direct service provision as an individuals
  • Be able to recognise priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs with confidence when appropriate
  • Beable to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
  • Assistpatients in decisions about prescribed medicines and supporting adherence as per NICE guidance
  • Mature and non-judgemental attitude and be flexible and adaptable
  • Self motivated with excellentcommunication skills
  • Understand the need for patient confidentiality
  • Ideally the suitable candidate will have a qualification from an accredited training programme for primary care based clinical pharmacists, which includes registration as an independent non-medical prescriber.
