Job Description

TristoneNash are pleased to be supporting a reputable Housing Association that is going from strength to strength as they look to recruit for a new service, working closely with their already established and award-winning Mental Health and Wellbeing team.We are looking for a Mental Health and Wellbeing Team Leader with passion and drive to join the team, enabling patients to thrive and work towards an independent future. This role will support delivery of a 24/7 support service in partnership with AneurinBevan University Health Board (ABUHB). The service will support patients transition from secure hospital residential settings into a supported community setting. The service will also aid the transition from CAMHS into Community Mental Health Teams. Eventually,the service will aid the individual to then transition into independence within the community.The support provided will be responsive to individual need and delivered in partnership with the Intensive Case Management Team at ABUHB. The service will be based between Blaina Gwent and Brynmawr, Gwent.As a psychologically informed service, delivery will be underpinned by the application of evidenced based practice, with a focus on using the principles of Structured Clinical Management (SCM). The service will be supported by the Intensive Case Managementteam at ABUHB to develop support plans which are underpinned by the principles of Structured Clinical Management (SCM).Accountabilities:

  • To be responsible for overseeing all Care Inspectorate Wales related governance and audit requirements, with the support of the Complex Needs and Specialist Support lead. This includes completion of weekly medication and paperwork audits and leading onsubsequent action points, preparing for Registered Individual and Care Inspectorate Wales audits, preparing reports for the Registered Individual and assessing staffs suitability as per Care Inspectorate Wales qualifications.
  • Be a role model for the services psychologically led culture, ensuring emotional, physical, and psychological safety of staff and service users in every contact.
  • Responsible for completion of overall service reports with the support of the lead.
  • Responsible for oversight of all service delivery actions and outcomes and consistently driving service improvement.
  • To lead on collecting, monitoring, and presenting service measurements and evaluations.
  • To lead on service promotion, presentation and sharing of good practice.
  • To assist the lead in monitoring the services budget, including sending invoices, reviewing monthly expenditure, and ensuring service efficacy wherever possible.
  • To lead on recruitment, staff training and staff performance support, where required.
  • Ensuring that the service is responding appropriately to all incidents and crises, ensuring that any relevant event is escalated to appropriate areas in a timely and effective manner, staff are supported throughout, and individuals safety is of paramountimportance.
  • To assist the lead with risk assessments, referral assessments, and transition of cases. The post holder must be comfortable to liaise at high levels with other agencies like crisis, Police, CMHTs, Hiraeth, local authorities and others.
  • To deliver quality training to the staff in support of the service lead, Hiraeth and the individuals specific needs.
  • Sharing on call responsibilities with wider management team. This includes providing direction, debriefs, instructions and reassurance where required. The post holder must be comfortable to manage emotionally charged situations and crisis in a psychologicallyinfluenced and safe manner.
  • To supervise seniors of the service in all areas. Responsible for the overall management of service staff.
  • Possess (or be willing to gain) Care Inspectorate Wales Values and Principles certificate and a Health and Social Care Level 5 NVQ.
Requirements:
  • Have an NVQ Level 5 in Health and Social Care, or be willing to work towards this whilst in post.
  • Understanding of PIE, ACEs, and strength/restorative-based approaches.
  • Experience of representing and advocating needs of service user and staff needs.
  • Experience of working with individuals with complex mental health needs, disrupted attachments and interpersonal difficulties and challenging behaviour.
  • You will have a good understanding of the wide range of community and third sector organisations operating across Gwent and be proactive in establishing links with these.
  • You must have a full driving licence and be prepared to travel as part of this role.
  • Although this role is generally Monday - Friday, 8am-4pm or 9am-5pm, you must be willing to work on-call or on the rota if the service requires it.
