Job Description

Are you looking to work with teenagers and adults We have an exciting opportunity for you to work in Adulthood Services.The pay rate would be up to £38 per hour depending on your location and this may be negotiable.Your key responsibilities will include:

  • To provide a timely transition for all Care Act eligible 14 to18 years old into adulthood and ensuring all 18 to 25 years old are receiving the appropriate ASC support under Care Act Eligibility Criteria.
  • To attend all 14 to 16 years old EHCP annual reviews once consent gained, to begin/review joint working with childrens services from 16 to 18 years old, and to take over all eligible support and funding for 18 to 25 years old.
  • To embed a reablement focus for the young adults.
  • To work with all families to ensure they understand the role and remit of PFA.
  • To develop close and effective relationships with providers, community and voluntary services, childrens services, schools and colleges.

  • Be Registered with Social Work England
  • Have a degree of equivalent in Social Work
  • Have an understanding of the Care Act 2014 and the Children and Families Act 2014
