Job Description

We are currently seeking dedicated individuals to join our hospital discharge team. If you are passionate about making a positive impact on the lives of adults in need, possess excellent assessment and coordination skills, and thrive in a fast-paced anddynamic environment, we invite you to apply and be part of our team, providing essential support for individuals transitioning from hospital to their community with dignity and care.Rate of pay - £30 per hourResponsibilities of this role:

  • Conduct comprehensive assessments of adult patients physical, emotional, and social needs to develop a deep understanding of their individual circumstances.
  • Coordinate and facilitate safe and timely discharge plans
  • Provide advocacy and support to patients and their families
  • Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process.
  • Collaborate closely with multi-disciplinary teams, including doctors, nurses, therapists, and community services, to ensure a holistic approach to patient care and effective coordination of services.
  • Maintain accurate and detailed documentation of assessments, care plans, interventions, and communication with patients and other stakeholders.
  • Assess and address any practical, financial, or housing-related needs that may impact the patients successful transition to community care.
Reed are now offering a joining bonus of up to£250 for Qualified Social Workers, terms apply.Requirements of the role:
  • Social Work England registered
  • 2 years experience in similar role
  • Must hold a UK driving license
