The team responds to safeguarding enquiries, carries out best interest assessments for service users, implementing and reviewing carepackages in the community (Service users homes / Supported Living / Care Homes).The successful applicant will work closely with the CCG, GPs, Community Health teams and voluntary sector agencies. We are looking for an enthusiastic social worker with skills and knowledge in the Adult Social care field, focusing on person centred andstrength-based approach in social care need assessment, support planning, mental capacity assessment and best interest decision making, S42 enquiry and DoLS challenges.Job Requirements:You will be a qualified, experienced Social Worker looking to progress your career or an accomplished Senior Social Worker, with a wide range of experience working with Older Adults and/or People with Disabilities. You will have developed your skills andknowledge with further post qualifying training and have specific experience with the relevant client group.You will be innovative and creative with a willingness to explore new models of working to improve the outcomes for our service users.
