Job Description

NHS TrustsCare Plan Prescribing Link WorkerPay Band - 5Base: Home remote working but may require occasional travel as required to hospital sites.Hours: Full-time 37.5 hours per weekDays: Monday - FridayContract: 3-4 months with the possibilityof this being extended to 12 months for the right candidate.Job Purpose

  • To enable patients access to the one-off Personal Health Budget pathway to allow them to take control of their health and wellbeing through linking them to services and support.
  • To work with the hospitals and Norfolk Trusts as a key part of the multidisciplinary team to provide a social prescribing service that will connect patients to services and support that will enable them to maintain their health and wellbeing or supportbehaviour change to achieve a healthier lifestyle.
Main Duties and Responsibilities
  • Take referrals from acute hospitals, community hospitals, local authority, Norfolk & Suffolk Mental Health foundation trust & other members of the multi-disciplinary team.
  • Provide personalised support to individuals that will enable them to take control of their health and wellbeing.
  • Work with professionals to co-produce a simple personalised care and support plan.
  • Where appropriate, refer the patient back to health professionals/agencies.
  • Work with a diverse range of people and communities and support them to receive social prescribing referrals.
  • Identify local community assets and gaps in local provision.
  • Provide non-clinical and clinical staff with information relating to what services are available within the community and how and when patients can access them.
Provide Personalised Support
  • Meet patients on a one-to-one basis (virtually), making home/hospital visits where appropriate and within agreed policies and procedures.
  • Be a friendly and engaging source of information about health, wellbeing and prevention approaches.
  • Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
  • Work with the person, their families and carers and consider how they can all be supported through social prescribing.
Caseload Management
  • Proactively manage a delegated one-off Personal Health Budget patient caseload.
  • To liaise with referrers, professionals & practitioners to complete a Personalised care and Support plan.
  • Escalate appropriately to relevant clinical team lead/head of services for referring organisation Healthcare.
To be considered for this role, you must have:
  • Previously worked in similar position within the VCSE Sector/Local authority/NHS
  • Comprehensive knowledge of health information systems
  • A good understanding of the health and social care environment and roles and responsibilities within it
  • Clear communicator with excellent writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
  • Ability to pull together comprehensive draft reports, data and letters Negotiating, networking and persuasive skills.
