Job Description

Support WorkerIn the communityWe are currently seekingtwo Support workers to join our Connect Team at Stapleford House,to help us provide early intervention support to adults in the community. You will be working with customers in the Rushcliffe, Gedling and Broxtowe areas of Nottinghamshire.You will therefore be required to work flexibly to ensure individual needs are met while maximising independence, choice, dignity, privacy and well-being. It is essential that you have the ability to travel independently.This service provides short term support to customers to enable them to remain independent and self managing in their own homes. The focus is on preventing or reducing the need for involvement by statutory services.Working 37.5 hours a week, Monday to Friday 9am5pm in the main, however you will need to be flexible in your working to meet customers needs on occasion.Key responsibilities will include, but are not limited to:

  • Provide assistance with the support needs of customers as identified in their support plan.
  • Encourage and support customers to establish and maintain positive relationships with their family and friends and engage in activities which help them to achieve their personal goals.
  • Provide support and practical assistance to customers with benefits, budgeting, correspondence and accessing appropriate services.
  • Provide social support to customers in line with policies & procedures and best practice.
  • Promote self advocacy and act as the customers advocate where appropriate.
  • Be aware of customers wellbeing, safety and state of health, reporting any concerns.
  • Provide care and support that is appropriate to the age, gender, disability, race, religion and sexuality of the customer to improve their quality of life and independence.
  • Ensure all necessary records are accurate and up to date.
As a lone support worker experience would be an advantage, but not always essential. To be successful in your application this should be clearly demonstrated in your CV.To be considered for this role your CV will need to reflect: -
  1. Experience of lone working so you are taking full responsibility for your work
  2. Experience of working with vulnerable people and supporting their independence
  3. Abilityto preparereports/records accurately
  4. IT literacy
  5. Valid driving licence
