Job Title: Patient Pathway CoordinatorDepartment: Division of JRU & CancerGrade: AfC Band 4Hours: 37.5 hours per week (£25,676 - £27,941 p.a pro rata. inc HCA)Responsible to : Team leaderAccountable to: Service ManagerOur busy Joint Reconstruction and Sarcoma Scheduling Team is looking for a Patient Pathway Coordinator to join the team. This broad, patient centered and high profile role is integral to the operational running of the department which is currently striving to meet the challenge of additional demand across the service. The main function of the role will be to coordinate and schedule the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the processThe ideal candidate will have significant experience of working in the NHS. The post holder will have an increasing responsibility for the development, coordination and facilitation of the scheduling process, will ensure all patients are robustly tracked through the appropriate pathway and ensure that all the relevant data is accurately recorded, reported and analysed in a timely manner.
