Job Description

Job overviewThe Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership FoundationTrust (MPFT).The service seeks to offer rehabilitation programme for a time limited period to rebuild functional range of movement, confidence, relearn new skills by promoting peoples independence through health & social care interventions. The Home First Integratedservice operates from 7am to 10pm over 365 days per year.The post holder will be responsible for working in close collaboration with the Home First Multi-disciplinary Team which includes Nurses, Therapists, Care Support Workers and Social Workers. Thers.The post holder will have exemplary communication skills, enabling them to be a role model for others, as well as positively representing the Trust values, in the delivery of effective, efficient and high quality services for service users. The post holderwill be enthusiastic, passionate and a dynamic individual.We are looking for an individual who is passionate about their delivery of care to patients who are in their own homes or residential homes and be responsible for a defined population of service users in the community across Staffordshire.For further information about this post please contact:Marie Allen Integrated Service ManagerMain duties of the job

  • The post holder will be an experienced clinician who will support the provision and provide clinical leadership to the North Home First Teams.
  • This leadership role will support and encourage the teams to provide high quality seamless provision, working flexibly across professional and organisational boundaries.
  • The post holder will provide clinical advice and guidance to the North Home First Teams, promoting and ensuring that good practice guidance is shared and adhered to include National Institute for Health and Care Excellence (NICE) guidance as well as theMidlands Partnership Foundation Trust policies and procedures.
  • The post holder will participate in the areas of service improvement relating to aspects of clinical practice for the North Home First Teams within MPFT, participating in elements of service redesign; supporting team development at all levels and developmentof systems to measure improvements in service delivery.
