Job Description

  • Annual Salary: £38,000 basic + commission (OTE £44,000)
  • Location: Monmouthshire
  • Job Type: Full-time, Permanent (Part-time positions available pro rata)
We are seeking a Qualified Social Worker to join our innovative care and support provider, focusing on arranging safe and timelydischarges for elderly patients. This role is crucial in supporting individuals transitioning from hospital to home, interimrespite, or residential care. The ideal candidate will be passionate about providing care and have experience working with discharge teams and local authorities. Day-to-day of the role:
  • Responsible for the discharge of patients, ensuring suitable services are in place to meet their needs.
  • Manage your own caseload, coordinate and monitor services effectively, and identify any gaps.
  • Maintain a range of resource information and develop links with other multi-disciplinary teams.
  • Manage statutory requirements under relevant acts and safeguarding protocols.
  • Conduct safeguarding and incapacity assessments in a fast-paced environment.
  • Adhere to strict deadlines and confidently assess a range of factors, including need, risk, and rights.
Required Skills & Qualifications:
  • Post-qualifying experience within elderly care/reablement/hospital discharge
  • Registered with Social Care Wales.
  • Degree in Social Work.
  • CPD requirements up to date.
  • Enhanced Disclosure and Barring check will be required.
  • Driver with access to own car (travel is essential).
  • Eligibility to reside and work in the UK.
