Job Description

Position: Support WorkerJob Hours: Full Time, Part Time and BankPurpose of PositionTo meet the physical, emotional, intellectual and cultural needs of the service users using a person centred approach. To promote and respect the service users right to choice, independence and control.To create and maintain good professional relationships with service users, their family and friends whilst maintaining good professional relationships with professionals/agencies.To create and maintain good professional relationships with colleagues.To adhere to all statutory obligations and Transparent cares’ policies, procedures and guidelines.To represent Transparent Care, promoting a positive, personal and professional profile, ensuring the good reputation of Transparent Care at all times.Principle Responsibilities

  1. To know, understand and undertake the tasks detailed in the service users support and care plans using a person centred approach
  2. To provide input into the care plans in consultation with the service user, other care staff and managers, family and other professional as appropriate.
  3. To know and understand and adhere to any risk assessments and Health and Safety procedures
  4. To assist service users with all personal care needs including undressing, washing, bathing and toileting needs As well as mobility problems and other physical disabilities
  5. To engage in the promotion of mental and physical well-being of service users
  6. To assist service users who may use challenge as a form of communication to feel understood and listened to.
  7. To maintain a professional and calm demeanour
  8. To provide, for and with the service users, household, domestic duties as detailed in the support plan or instructed by management
  9. To assist service users in the correct administration of their medication
  10. To comply with the organisation’s policies and procedures at all times
  11. To encourage service users to remain as independent as possible
  12. Attend mandatory training where appropriate
  13. To enable and assist service users to maintain on a day to day basis as much autonomy and independence as possible.
  14. Attend reviews of support as appropriate.
  15. To act at all times, to safeguard service users from any form of abuse and to report such concerns immediatel
