Job Description

Swansea COPD Early Discharge SchemeWe are looking for dynamic, visionary and hard-working candidates to join our expanding Respiratory Department. We are advertising 2 full time posts for excellent candidates to develop a COPD early discharge scheme. The team consists of a lead Band 7 specialist nurse ,a Band 7 Physiotherapist and 4 Band 6 nurses The team will be responsible for identifying and assessing inpatients at both Morriston and Singleton Hospitals. Suitable in-patients with an exacerbation of COPD who are from the Swansea area will be supported at home by this team.The team are about to extend into Admission Avoidance working with GP PracticesThe successful candidates will be expected to work with colleagues to deliver a unified respiratory service across Swansea Bay UHB.Swansea Bay UHB covers a population of around 390,000 in the Neath Port Talbot and Swansea areas and we have a budget of around £1bn. The health board employs approximately 12,500 staff.The Health Board has close links with Swansea University (with its graduate entry medical school) and other University College partners in South Wales especially in areas of research & development and multi-professional teaching. Consequently, there are great opportunities for research and teaching.Staff in the Health Board benefit from a flexible approach to working practice. We place great emphasis on continuous professional development and will actively support you in the achievement of personal goals.
