Job Description

HRGO are recruiting for a Hybrid Support Coordinator - for our client based in Sandwich. They are helping Carers who are based in East Kent and the post holder may need to occasionally travel across this area to the East Kent hospitals or Carer Support groupsor events or Carers home. The postholder will be primarily working with taking incoming calls in the Hub team (calls from Carers or professionals) but will also be required to work with several departments in the organisation, subject to demand. The postholderwill be a point of contact for Carers to provide up to date information and support, assessing Carers to support them in their caring role.Core Duties:

  • Provision of a telephone triage, assessment and support service to Carers from our Carers Hub
  • Respond appropriately to calls received in the Carers Hub, directing the caller to the most appropriate team member
  • Manage incoming referrals, ensuring procedures are followed and information systems are updated
  • Provision of information to Carers and professionals over the telephone and by e-mail
  • Provision of emotional support (not counselling) to Carers
  • Work with Carers to identify their needs and appropriate support options to meet needs. This could involve visiting any of the hospitals in the East Kent area to talk to Carers.
  • Take into account Carers views and feelings in relation to their care, support and treatment, or that of the person they care for
  • Build a rapport with Carers and maintain contact using their preferred communication method (including telephone, email, skype, face to face)
  • Empower Carers by supporting them to develop confidence and skills to encourage self-advocacy
  • Undertake assessment reviews which could include home visits to Carers to assess their telecare and equipment needs.
  • Attend support groups or events to facilitate a session for Carers.
  • To help with follow up calls to Carers as part of the Carers Assessment process.
  • Liaise with and make referrals to other organisations on behalf of Carers
  • Assess Carers eligibility to access funded support, such as the Clinical Commissioning Group (CCG) funded Primary Care Rapid Access Service and grants from Carers Trust and other charities
  • Arrange and coordinate support services for Carers through the Carers Hospital Discharge Service as required to support Carers Respite Coordinator
  • Manage bookings for Carer events and activities and organise transport and respite to facilitate attendance
Essential
  • Good knowledge of the needs of Carers and those with care needs
  • Experience of using Microsoft Office programmes and database or information systems
  • An awareness of the voluntary and statutory sectors
