Job Description

An exciting opportunity has arisen to join the existing West Kent Covid Oximetry @ Home and newly established Post Covid Assessment Service (PCAS) for Kent and Medway working with multidisciplinary colleagues.You will work supporting patients during their period of Covid illness and recovery from Long Covid using remote monitoring technology and implementing the use of the ‘Your Covid Recovery’ app. Working through Multi-Disciplinary Team Meetings you will be involved with other professionals in the decision-making regarding ongoing support for individuals with Long COVID symptoms.In accordance with the local guidance, the post-holder will undertake clinical sessions mainly within the Oximetry@Home and at times PCAS dependent on patient needs and the demands of the service. The Clinical Support Worker will be responsible for co-ordination of on boarding new patients ensuring follow ups are carried out effectively and efficiently. The Clinical Support worker will work with a qualified healthcare professional to enable supervision and advice as required.This role is fixed term until End June 2021 with a view to extend.West Kent Primary Care is an established employer and service provider in the West Kent area.
