Job Description

Hospice in the Weald provides compassionate, personalised and holistic care for everyone in our community living with a terminal illness, and for those important to them. We are inviting passionate and like-minded Nurses to join our dynamic TeamWe are seeking an experienced Advanced Nurse Practitioner (ANP) to work within the Hospice in the Home team to provide outstanding care to patients and those important to them. Our Hospice in the Home team deliver highly effective symptom management and interventional support to patients with a palliative diagnosis, in their own homes, community, private hospitals, care homes and at the Hospice.Reporting to our Medical Director you will help to bridge the gap between primary care and Hospice palliative care by working within GP surgeries helping to identify patients for early palliative care support.You will be an expert nurse with advanced assessment and clinical decision-making skills, and you will be able to contribute to the development of clinical practice across all aspects of palliative care.You will be able to undertake complex and sensitive holistic care assessments and plan, implement and evaluate care, utilising advanced communication, clinical reasoning, diagnostic and prescribing skills.You will be able to make a significant contribution to the development of Hospice services and the wider Hospice clinical offering.   • Registered Nurse Level 1
  • Palliative Care Qualification at degree level
  • Good Clinical Practice (GCP) qualification
  • Teaching Qualification
  • Clinical Assessment Skills
  • Non-medical prescribing qualification (or willing to work towards this within one year of appointment)
  • Master’s Degree (or equivalent experience & willing to attend modules as appropriate)
  • Experience required:
  • Significant and recent experience in Oncology/Palliative Care/End of Life Care
  • Recent CNS or community experience at a senior level managing large caseloads effectively
  • Experience of team working and skill mix utilization to improve patient outcomes
  • Experience of working in settings other than Hospices (Acute Trust, Community, Care Homes)
