An exciting opportunity has arisen for a new, permanent role as Associate Director of Health Improvement and Place across the North & Mid Hampshire Integrated Care Partnership (ICP); a core delivery function of the wider Hampshire and Isle of Wight Integrated Care System (ICS).As we continue our journey towards driving patient centred and place based care for our patients we have identified the need for a new member of the Senior Leadership Team to focus and drive place based health improvement across our local communities.The role will require innovative thinking and a strong partnership focus in order to build and maintain long term, meaningful relationships between local government and health stakeholders. The successful candidate should be a strategic leader able to demonstrate innovative thinking and accomplished outcomes through collaborative working across a range of partners.You should have a keen interest in Health inequalities and developing communities using Population Health Management approaches.
