Job Description

Torfaen, WalesCirca £20,700 per annum pro rataPermanent, Part Time working 24 hours per week, Monday to Thursday 9am to 3:30pmGiving children and young people the foundations, they need to thrive.The roleTafarn Newydd Families First (0-4) At Tafarn Newydd, we are a service that provides a multi-agency child & family centred approach which promotes and enhances the development of disabled children, aged 0 - pre-school. We receive referrals from professionals who have identified a need of our service, we then undertake an assessment that identifies areas of child development, support for the family, and includes safeguarding issues. The service promotes the childs; linguistic, cultural, physical, social, emotional, and cognitive development through assessment and individual play plans that address their specific needs. This is a child focused service with all activity centered on specialized play provision with parental involvement and support at all stages using person centered planning.As a Children and Families Practitioner, you will be joining an established team and providing support for the child and family in play and development sessions and then a smooth transition into education. You will make a difference by:- Building and maintaining a strong partnership with parents and carers and to work with these and other multiagency professionals.- Working alongside the Manager and the rest of the team to ensure the nurserys philosophy is fulfilled.- Providing a high quality service to children and their families.- Delivering complex and specialist interventions with children or families on a 1:1 basis, group or community setting.- Undertaking assessments of all cases as well as reviewing and managing risks.- Ensure safeguarding practices are in place at all times.- Being flexible within the early years setting including, preparing snacks and to work within a wider team that operates within the building and community.- Personal care, you must be willing to undertake tasks such as promoting self care skills which will including attending to their personal care/changing.Also you must be willing to undertake relevant training as some of our children have complex health needs and require interventions such as PEG feeding, epilepsy and put this into practice in line with the childs care plan.You will need:- NVQ3 in Health and Social care, playwork, early years or equivalent is Essential.- Effective team working skills and experience of working in a team.- Excellent communication skills.- Be able to demonstrate experience of working with children with various disabilities.- Good knowledge of safeguarding.- A positive and proactive approach towards work.
