This role involves the care of vulnerable adults with complex mental and physicalhealth needs. Cornerstone Healthcare is committed to safeguarding andpromoting the health and welfare of our residents. All staff are expected to sharethis commitment.The rate of pay in £19.20 with a paid break. All positions are FULL TIME and PERMANENT. This consists of 3 shifts the first week and 4 shifts the second week, continuing in this pattern and would be expected to work some weekends. We would consider RGNs or RMNs.Summary of Responsibilities: To liaise with all members of the team and key stakeholders including family,friends, visiting professionals and commissioners to ensure holistic care of ourresidents. To assess, develop plans and implement plans of care for effective nursingcare. To support senior nursing staff in providing direction, supervision andleadership of team members of the lodge/wing to include direct linemanagement of senior carers To provide clinical leadership within the Lodge/Wing on a daily basisMAIN DUTIES1. CLINICAL Participate in the assessment planning, implementation and evaluation ofprogrammes of care in consultation with residents and family. To demonstrate clinical decision-making skills, displaying evidence-basedpractice. Make effective clinical judgement affecting safe and timely delivery ofresident’s care. Be competent in the safe use of clinical equipment to deliver effectiveresidents care. Maintain accurate written records at all times. Participate in the setting and maintaining of professional standards on theLodge/Wing and environment. Engage and interact with carers/ family, encourage them to be activelyinvolved in the caring process. Supervise and instruct senior carers and HCA’s to ensure they integrateinto the team and operate effectively within the boundaries of their role. To ensure that residents’ safety is maintained and that risk managementsystems are utilised correctly Organise and delegate workload to ensure effective delivery of resident’scare is carried out by appropriate skilled members of staff. Assist in the control and use of resources within defined area ofresponsibility. Develop Link Role within the team.2. KEY RELATIONSHIPS To develop a rapport with the residents and their families throughout theirstay encouraging them to be actively involved in their care planning anddischarge planning. To communicate effectively with all members of the team, setting realisticresidents’ goals.________________________________________________________________________ To support the local GP’s who provide the medical cover to the service To develop links with the support services within the local area To develop good working relationship with referrers e.g hospitals to ensurea safe and smooth transition for our residents.3. STAFF LEADERSHIP AND MANAGEMENT To demonstrate an up-to-date knowledge of local and national issues To be actively involved in the Cornerstone appraisal system (PersonalDevelopment Review) To support new staff, taking an active role in meeting their orientation needs To actively participate in Lodge/Wing meetings To assist senior staff in the day-to-day management of the Lodge/Wing andbe able to take responsibility for the Lodge/Wing/ unit in the absence of amore senior member of staff. This will include maintaining a safe level ofstaffing on the Lodge/Wing and contacting the on-call managers whenappropriate according to the To assist in the deployment of resources to meet the needs of the resident’scondition, dependency and special requirements. To be actively involved in health and safety requirements of the Lodge/Wingincluding COSHH and manual handling. To participate in clinical audit.4. GENERAL REQUIREMENTS Follow all Cornerstone policies and procedures particularly those in in relation toinfection - prevention and control Are aware of their responsibilities in relation to safeguarding vulnerable adults,and the specific responsibilities placed on individuals who care for suchresidents. Adhering to any relevant safeguarding policies and acting in a waythat safeguards the health, safety and well- being of vulnerable adults at alltimes. Comply with the Cornerstone policy on confidentiality, and the Data ProtectionAct 1998 as amended and GDPR, relating to information held manually or oncomputerised systems.Respect the confidentiality and privacy of residents and staff at all times, Maintain a constant awareness of health, welfare and safety issues affectingcolleagues, residents, visitors and themselves, reporting any accidents or fault inline with Cornerstone policy, Participate in statutory/mandatory, personal training, development, appraisal, andattend all relevant training courses as required.
