Clinical LeadPulse Nursing at Home (part of Acacium Group)Homebased with occasional field based travelPermanent, Full Time£42,000-£45,000 per annum + £3000 car allowance &up to 5% annual bonusUnlock your potential:Due to business growth, we have a fantastic job opportunity for a Clinical Lead to join our team where you will have the flexibility to work on a remote basis with occasional visits to one of our local offices.We are excited to announce our new Short Term Reablement Service, which will see thousands of elderly people across the UK placed quickly and efficiently back into the comfort of their own homes with world-class temporary everyday care. This is an incredibleand fast-growing service so we are looking for the best talent to run it!As our Clinical Lead you will coordinate and facilitate the safe and effective discharge of patients from hospital, supporting the proactive reduction in delayed transfers of care. Your role will see you providing assessments for intermediate care servicesand working proactively within integrated health and social care teams to promote channels of communication both within the team, CCG and wider community.Part of Acacium Group, Pulse Nursing at Home provides flexible, bespoke care for people living in their own homes and communities.   • Manage a wide variety of caseloads
  • Participate in daily board rounds to coordinate safe, effective discharges of patients and promoting patient flow
  • Take responsibility for coordinating the team, appropriately delegating and prioritising workloads and attend capacity meetings.
  • Assist in dealing with complaints in the first instance and act as an advocate of patients

  • A valid NMC registration
  • Nursing experience within the community
  • Experience participating in a research or audit project and implementing full audit cycle
  • Demonstrable contribution to practice developments in primary care/critical care/complex care
